الإجابة: B. Admit to ICU and give IV artesunate
Fever after travel to India with hemolysis (low Hb, high LDH), thrombocytopenia and a raised creatinine (renal dysfunction) = severe malaria. Severe malaria needs urgent hospital admission and IV artesunate. Chloroquine and oral atovaquone-proguanil are for uncomplicated malaria, and empiric antibiotics do not treat malaria.

His kidney involvement marks severe malaria with organ dysfunction. In the notes chloroquine appears only as the regimen for chloroquine-sensitive malaria, not as treatment for this severe picture.

B
ليش صح
Severe malaria needs urgent hospitalisation and IV artesunate 2.4 mg/kg at 0, 12 and 24 hours, then daily until oral therapy is possible. His raised creatinine with hemolysis and low platelets after travel to India is the severe picture this applies to.

C
ليش غلط
Atovaquone-proguanil appears in the notes as an alternative to first-line artemether-lumefantrine for chloroquine-resistant malaria. His kidney involvement marks severe malaria with organ dysfunction, which is not what these regimens are listed for.

D
ليش غلط
Malaria is a potentially life-threatening disease caused by Plasmodium parasites, so empiric antibiotics would not treat it and would only delay antimalarial therapy.
