الإجابة: A. Addison's disease
Fatigue, generalised hyperpigmentation, hypoglycaemia and high ACTH = primary adrenal insufficiency; anti-21-hydroxylase antibodies mark autoimmune adrenalitis, the commonest cause of Addison's disease in industrialised countries. Congenital adrenal hyperplasia is an inherited 21-hydroxylase enzyme deficiency presenting in infancy, not an antibody-mediated adult disease.

A
ليش صح
High ACTH marks primary adrenal insufficiency, and anti-21-hydroxylase antibodies are sought in primary disease because they mark autoimmune adrenalitis (Addison).

ليش غلط
The notes list 21-hydroxylase deficiency as an enzyme defect of cortisol synthesis; anti-21-hydroxylase antibodies instead point to the idiopathic (autoimmune) form, the most common type of Addison disease in the industrialized world.
