A 30-year-old man has 3 months of intermittent breathlessness and expiratory wheeze, worse with exercise and at night. No smoking or occupational exposure. Spirometry (pre- and post-bronchodilator) is normal. Which test would best confirm asthma?

الإجابة: A. Methacholine challenge test
Spirometry can be normal between symptoms. When asthma is suspected but PFTs are nondiagnostic, a bronchoprovocation (methacholine challenge) test shows airway hyperresponsiveness, with an FEV1 drop of more than 20%.
- The asthma lecture says peak flow meters are not diagnostic; they are for monitoring exacerbations and follow-up. (One AMBOSS-style summary does allow PEF variability when spirometry is normal.)
- Chest X-ray and eosinophil count are non-specific.

A
ليش صح
When initial PFTs are inconclusive (normal spirometry), a methacholine challenge is the bronchial challenge test used to confirm asthma: FEV1 falls by 20% or more.

B
ليش غلط
Chest X-ray is not routinely recommended in asthma; it is only used for differential diagnosis, and it is usually normal. It cannot confirm asthma.

C
ليش غلط
Eosinophilia is only a possible supportive finding among additional studies; confirmation needs symptoms, airflow limitation and excessive variability in lung function.
