الإجابة: C. IV vancomycin and cefepime
Pneumonia starting >48 h after admission is hospital-acquired (HAP); the main concern is Pseudomonas and Staph aureus (MRSA). Empiric therapy is therefore an IV anti-pseudomonal β-lactam (e.g. cefepime) plus an anti-MRSA agent (vancomycin). Clindamycin, daptomycin (inactivated by surfactant) and oral amoxicillin-clavulanate do not cover Pseudomonas.

A
ليش غلط
Clindamycin acts on gram-positive bacteria and anaerobes but has no gram-negative activity, so it cannot be an empiric HAP regimen on its own; its main role is as the alternative for Strep pharyngitis or skin infections in penicillin allergy.

B
ليش غلط
Daptomycin is not recommended in pulmonary infections because pulmonary surfactant inactivates it.

C
ليش صح
HAP combination therapy pairs an anti-MRSA drug (vancomycin or linezolid) with an antipseudomonal beta-lactam such as cefepime.

Oral amoxicillin-clavulanate is the choice for sinusitis, otitis media and dental infections. HAP (Pseudomonas, Staph aureus) needs IV anti-pseudomonal plus anti-MRSA therapy.
