الإجابة: C. Serum lactate level

✓ مطابق للمصدر Shock.pdf · p. 5

Serum lactate measures tissue hypoperfusion (lactate >2 supports shock). Under the Surviving Sepsis targets, one endpoint of resuscitation is a normalised lactate, so repeating the lactate after 6 hours shows whether perfusion is improving. Blood cultures identify the organism; they do not track the response to therapy. WBC changes too slowly and too non-specifically to judge early improvement. Haemoglobin does not reflect sepsis resolution.

ليش كل خيار صح أو غلط

  • A ليش غلط

Blood cultures are taken to identify the responsible pathogen (with Gram stain), not to follow how the patient is responding, so they do not show whether resuscitation is working.

  • B ليش غلط

WBC is only a nonspecific CBC finding in sepsis (leukocytosis or leukopenia) and a SIRS criterion, so it is a poor marker of early response to treatment.

  • C ليش صح

The Surviving Sepsis end-points of resuscitation include normalised lactate, so repeating the lactate after treatment shows whether perfusion has improved.

    ✓ Shock.pdf · p. 5
  • D ليش غلط

Anemia is just one nonspecific CBC finding in sepsis, so the hemoglobin level does not tell you whether the infection or perfusion is improving.
