الإجابة: C. azithromycin
Insomnia, nervousness and palpitations in a patient taking theophylline suggest theophylline toxicity, a narrow-therapeutic-index drug. Macrolides (erythromycin, clarithromycin, azithromycin) inhibit hepatic drug metabolism and prolong the action of theophylline, so azithromycin started for pharyngitis is the cause. Penicillin G and doxycycline do not have this interaction. Clarithromycin/erythromycin are the strongest culprits, but azithromycin is the only macrolide offered.
