الإجابة: C. Peripheral blood smear
Paroxysmal fever with chills recurring every other day (48 h, tertian), splenomegaly and travel to an endemic area (Jizan) suggest malaria. Diagnosis is by peripheral blood smear (thick smear best initial test, thin smear confirms species). Empiric antibiotics or blood culture would miss the diagnosis.
