الإجابة: D. Early morning serum cortisol and ACTH
A TB patient with weakness, weight loss, hypotension and pigmentation over the elbows most likely has adrenal TB causing primary adrenal insufficiency. Early-morning serum cortisol and ACTH is the best initial test (low cortisol, high ACTH). CBC, ESR and blood cultures are non-specific and would not explain the pigmentation.

A
ليش غلط
A CBC can show eosinophilia in adrenal insufficiency, but that is only a general finding; the best initial test for hypocortisolism is a morning serum cortisol (or ACTH) level.

Cortisol is the initial test (low in adrenal insufficiency) and ACTH shows the cause (high in Addison disease, the primary form).
