الإجابة: B. Pheochromocytoma
A background of hypertension, headaches and 'anxiety' followed by a sudden hypertensive surge during surgery/anaesthesia suggests an undiagnosed pheochromocytoma: surgery and tumour manipulation trigger catecholamine release. Two beers a day does not make alcohol withdrawal likely at induction, and anaphylaxis causes hypotension, not hypertension.

B
ليش صح
Surgery or abdominal manipulation compresses an adrenal tumour and releases catecholamines, causing a sudden hypertensive crisis in an undiagnosed pheochromocytoma.

C
ليش غلط
Anxiety, sweating and a feeling of impending doom are themselves part of the pheochromocytoma presentation, together with episodes of severely elevated BP, so the sudden surge points to the tumour.
