الإجابة: C. exogenous steroid use
Cushingoid features with low ACTH mean an ACTH-independent cause: either an adrenal tumour or exogenous steroids. Low DHEA (adrenal androgens suppressed) and a normal urinary cortisol favour exogenous steroids, since an adrenal cortisol-secreting tumour would raise urinary free cortisol. Cushing disease and ectopic ACTH would have high ACTH. ('Adrenal glucose' is presumably a typo for adrenal adenoma.)

-
A
ليش غلط
Cushing disease is an ACTH-secreting pituitary adenoma, so ACTH would be raised; his ACTH is low.

C
ليش صح
Exogenous steroids suppress adrenal androgen production (low DHEA) while causing Cushingoid features; it is also the most common cause of hypercortisolism.

D
ليش غلط
Ectopic ACTH syndrome is an ACTH-secreting tumour outside the pituitary driving cortisol, so ACTH would be raised; his ACTH is low.
