الإجابة: D. Sleep study
Obesity, loud snoring, daytime sleepiness and a crowded upper airway point to obstructive sleep apnoea, which is linked to insulin resistance/T2DM and is one of the commonest causes of secondary (resistant) hypertension through sympathetic activation during apnoeas. A sleep study (polysomnography) is the diagnostic test. Ambulatory BP monitoring would only confirm the hypertension, not explain it; cortisol, TSH and renal Doppler chase other causes the history does not suggest.

C
ليش غلط
Ambulatory BP monitoring only measures the BP; OSA is one of the most common causes of secondary hypertension, so it explains the poor control.
