The following are a patient ' s arterial blood gas (ABG) and basic metabolic panel, test results. pH 7.20, pC02 60mmHg, Bicarbonate 32mEq / L, Sodium 140mEq /L, Chloride 100mEq / L. Which of the following is the best interpretation of these values?

الإجابة: A. Respiratory acidosis with metabolic compensation
Low pH with PCO2 60 = primary respiratory acidosis. HCO3 32 is raised by ≈ 4 per 10 mmHg PCO2 rise, i.e. the expected chronic metabolic (renal) compensation. The anion gap = 140 − (100 + 32) = 8 (normal), so there is no AG acidosis (B), and C/D are wrong because the patient is acidaemic.

-
A
ليش صح
High PCO2 with low pH is respiratory acidosis, and the HCO3 of 32 is the expected rise (about 4 per 10 mmHg of PCO2), i.e. metabolic compensation.

B
ليش غلط
The anion gap is Na − (Cl + HCO3) = 140 − 132 = 8, within the normal 5 to 15, so there is no widened-gap metabolic acidosis.

C
ليش غلط
pH 7.20 is acidosis (pH < 7.35), not alkalosis, so a respiratory alkalosis is excluded.
✓ ABG & A-a gradient.pdf · p. 1 -
D
ليش غلط
Alkalosis needs a high pH (> 7.45); this patient's pH is 7.20, which is acidosis.
