الإجابة: E. Pulmonary embolism
Major lower-limb orthopaedic surgery (hip/joint replacement) is one of the strongest VTE risk factors, so chest pain a week later is pulmonary embolism until proven otherwise. Fat embolism follows long-bone fracture within days and adds confusion and petechiae; MI, pneumothorax and pneumonia have no specific link to this setting.

Pneumothorax is one of the alternative causes of hypoxia looked for on chest X-ray when PE is suspected, not the leading diagnosis after major surgery.

C

Fat embolism follows long-bone fracture, usually within days of the trauma, and presents with dyspnea, neurologic findings and a petechial rash; none of these is described here.

Pneumonia is one of the alternative causes of hypoxia (PNA) looked for on chest X-ray when PE is suspected, not the leading diagnosis after major surgery.

Major lower-limb orthopedic surgery (joint replacement, hip fracture surgery) is a listed risk factor for venous thrombosis, so chest pain after hip replacement points to PE.
