الإجابة: B. Pulmonary embolism
Sudden dyspnoea with haemoptysis after a long-haul flight (venous stasis) with a normal chest X-ray points to pulmonary embolism. CXR is usually normal in PE, whereas pneumonia would show consolidation. Bronchitis and unstable angina do not explain haemoptysis plus the travel risk factor.

-
A
ليش غلط
Acute bronchitis has a gradual onset (like TB), whereas PE presents suddenly; her dyspnea suddenly worsened.

B
ليش صح
PE presents with pleuritic chest pain, SOB and hemoptysis, which matches her sudden worse dyspnea with hemoptysis after a long flight.

C
ليش غلط
Ischemic heart disease is listed as a cause of dyspnea with chest pain; it does not explain her hemoptysis after a long flight.

D
ليش غلط
Pneumonia would be the alternative cause of hypoxia found on chest X-ray; in PE the CXR is usually normal, as in this patient.
