الإجابة: C. Sputum for Acid Fast Bacilli
Chronic productive cough with haemoptysis, night sweats, fever, weight loss and upper-zone shadowing = suspected active pulmonary TB. The diagnostic test is sputum for acid-fast bacilli (≥3 samples, with culture). TST/IGRA only screen for latent TB and are not used in a symptomatic patient. CT and sputum cytology do not confirm TB.

A
ليش غلط
TB is diagnosed by microbiological tests (direct visualisation, culture, genetic detection), not by chest CT, which only shows suggestive findings.

B
ليش غلط
Cytology looks for malignant cells, whereas acid-fast bacilli is the TB test; cytology would not confirm TB.

For a symptomatic patient with suspected active pulmonary TB the diagnosis is sputum AFB; PPD and IGRA are screening tests, not the right choice here.
