A 60-year-old female presented to emergency department with 5 days of progressive dyspnea and cough. Chest x-ray showed moderate right pleural effusion. Thoracentesis was done and pleural fluid showed LDH: 600 U/L, Protein 56 g/L, WBC 5700/uL with lymphocytes predominate. Serum LDH 90 U/L, serum protein 72 g/L. Which of the following would explain the underlying etiology of this effusion?

الإجابة: B. Malignancy
Protein ratio 56/72 = 0.78 (>0.5) and LDH ratio 600/90 = 6.7 (>0.6) make this an exudate, and it is lymphocyte-predominant, which fits a malignant effusion. Hepatic hydrothorax, CHF and renal failure cause transudates.

B
ليش صح
Protein ratio 0.78 and LDH ratio 6.7 make this an exudate, and a lymphocyte-predominant exudate is typical of malignant pleural effusion.

Congestive heart failure is a transudative cause, but this fluid is an exudate.
