الإجابة: C. Discharge on doxycycline
A healthy 24-year-old with lobar CAP, normal vitals apart from mild fever, GCS 15 and normal renal function has a CURB-65 of 0, so outpatient treatment is appropriate. For previously healthy outpatients, empiric monotherapy is amoxicillin or doxycycline (or a macrolide). Microbiological tests are not needed for outpatients (A). Discharging without antibiotics (B) leaves the pneumonia untreated. A respiratory fluoroquinolone (D) is for patients with comorbidities or risk factors for resistance.
