He was well until 3 days earlier, when he experienced the onset of nasal congestion, mild sore throat, and a cough with the production of a small amount of clear sputum. The patient smokes one pack of cigarettes daily. The temperature is 38.3°C, blood pressure is 130/86 mmHg, pulse is 110 bpm, and respiratory rate is 22 bpm. Pulse oximetry shows 92% oxygen saturation on room air. Chest X-ray showed right lower lobe consolidation. What is the most likely causative agent in his case?

✓ مطابق للمصدر (Y4)-(M B) Pneumonia and pleural effusion - Resident.pdf · p. 29

An otherwise healthy outpatient with acute fever, cough and lobar consolidation has typical community-acquired pneumonia. Streptococcus pneumoniae is the most common cause of CAP. Pseudomonas is linked to hospital-acquired pneumonia, bronchiectasis and cystic fibrosis. Staph aureus is classically secondary to influenza. E. coli is not a usual CAP pathogen in a healthy young adult.

ليش كل خيار صح أو غلط

  • A ليش صح

Streptococcus pneumoniae is the most common cause of pneumonia, including community-acquired pneumonia, which fits this healthy outpatient.

    ✓ (Y4)-(M B) Pneumonia and pleural effusion - Resident.pdf · p. 29
  • B ليش غلط

Pseudomonas aeruginosa is found in cystic fibrosis and bronchiectasis patients, which this healthy smoker is not.

  • C ليش غلط

Staphylococcus aureus is more common following an episode of influenza, but S. pneumoniae remains the most common cause of pneumonia, so it is the more likely agent in this otherwise healthy young man.

    ✓ (Y4)-(M B) Pneumonia and pleural effusion - Resident.pdf · p. 29
  • D ليش غلط

    E. coli is not among the listed CAP organisms (typical: S. pneumoniae, H. influenzae, M. catarrhalis; atypical: Mycoplasma, Chlamydia pneumoniae, Legionella).
