An HCT of 65% means secondary polycythemia from chronic hypoxemia, which is an indication for long-term oxygen therapy. LTOT is proven to lower mortality in COPD. LABA and ICS improve symptoms and exacerbations but not survival. Vaccines are recommended, but Step Up lists only smoking cessation and home oxygen as consistently lowering mortality, and the HCT clue points to oxygen.
