A 59-year-old patient presented to the ER with a COPD exacerbation and was treated with bronchodilators, systemic corticosteroids, and IV antibiotics. The nurse notes that the patient has experienced a loss of consciousness and has become drowsy, prompting a call for reassurance. The Glasgow Coma Scale is 10/15, with pupils that are equal and reactive, and all four extremities move in response to pain stimulation. What is the next investigation that should be ordered?

الإجابة: A. ABG
A COPD exacerbation patient who becomes drowsy (GCS 10) with equal reactive pupils and no focal deficit most likely has hypercapnia / CO2 narcosis (often oxygen-induced). The next investigation is an ABG to measure PaCO2 and pH and decide on NIV. A non-contrast CT head is for focal signs or suspected stroke, which are absent here.

Drowsiness in a treated COPD patient suggests CO2 narcosis from oxygen-induced hypercapnia, and blood gas analysis assesses hypoxemia, hypercapnia and acidemia.
