A 33-year-old woman is being treated for asthma exacerbation with ongoing nebulization of albuterol, ipratropium bromide, and oral prednisolone with remarkable improvement in her symptoms. She is finishing a course of trimethoprim-sulfamethoxazole for cystitis. While you are rounding on her, the bedside nurse informs you the patient's potassium level is low 3.1 mmol/L (normal value 3.5-4.5 mmol/L). Which of the following is the best explanation for her electrolyte disturbance?

الإجابة: A. Albuterol
Albuterol (a β2-agonist) stimulates the Na+/K+-ATPase and shifts potassium into cells, causing hypokalaemia; this is why it is also used to treat hyperkalaemia. Repeated nebulisation in an exacerbation makes it the most likely cause. TMP-SMX does the opposite: it blocks the aldosterone effect in the distal tubule and causes hyperkalaemia. Ipratropium does not affect potassium.

A
ليش صح
Albuterol stimulates beta-2 receptors, which increases Na/K ATPase activity and moves potassium into cells, lowering serum K. High repeated nebulised doses explain her hypokalemia.

B
ليش غلط
Hypokalemia is listed among the main side effects of SABAs such as salbutamol (with tremor and palpitation); ipratropium, an anticholinergic, is not listed with it.

D
ليش غلط
TMP-SMX is listed among the causes of hyperkalemia (it reduces the aldosterone effect on the distal tubule), so it would raise, not lower, her potassium.
