A 50-year-old woman known to have type 2 diabetes mellitus, essential hypertension, and asthma presented to the Emergency Department complaining of headache. She reports that her home blood pressure readings have been consistently elevated. She ran out of her antihypertensive medications one week ago (Her previous medications included Losartan and a beta-blocker "not sure") On examination, her blood pressure was 188/120 mmHg. All laboratory investigations were within normal (including RFT and LFT). Which of the following best describes her current condition?

الإجابة: C. Hypertensive urgency
Her BP is ≥180/120 and all labs, including renal and liver function, are normal, so there is no target organ damage (TOD). That makes it hypertensive urgency. A hypertensive emergency (or malignant hypertension) needs evidence of acute organ damage, such as AKI, encephalopathy, ACS, pulmonary oedema, dissection or retinal injury. Headache alone does not count.
