الإجابة: B. Amlodipine + indapamide
BP 180/100 is far above target, so combination therapy of first-line classes is indicated; first-line classes are ACEI/ARB, CCB and thiazide(-like) diuretics such as indapamide. Amlodipine + indapamide is the only option combining two first-line agents. The others are poor pairs: verapamil + beta blocker causes heart block, nifedipine + nitrate causes severe hypotension, verapamil + diltiazem are the same class, and hydralazine is not first line.

A
ليش غلط
Vasodilators such as hydralazine are not commonly used; they are typically given with beta-blockers and diuretics in refractory HTN.

B
ليش صح
Amlodipine is a dihydropyridine CCB and indapamide a thiazide-like diuretic: two main antihypertensive classes, with none of the interaction warnings listed for the other pairs.

C
ليش غلط
Nifedipine plus nitrates is a listed drug interaction causing severe hypotension.

D
ليش غلط
Verapamil and diltiazem are both non-dihydropyridine CCBs, so this pairs two drugs of the same class rather than two different classes.

Verapamil plus a beta-blocker is a listed drug interaction causing severe cardiac depression (heart block).
