A 38-year-old Hajji patient presented to ER complaining of exertional dyspnea upon minimal activity, his past medical history is significant for Acute Rheumatic Fever. Upon exam: his PR 70 Bpm, BP 126/76, Temp. 37.2, his apex beat was tapping with diastolic murmur, opening snap and increasing with activity, what is the hallmark of the following disease?

الإجابة: B. Elevated atrioventricular gradient pressure
Rheumatic history, a tapping apex, opening snap and diastolic murmur = mitral stenosis. Its haemodynamic hallmark is an increased diastolic pressure gradient across the mitral valve (LA > LV). The LV is underfilled, not hypertrophied, and its diastolic pressure is not raised. MS leads to LA dilatation and RVH instead.

A

In mitral stenosis the LV is underfilled and it is the right ventricle that hypertrophies after pulmonary congestion, not the LV.

B
ليش صح
The hallmark of mitral stenosis is an increased mean diastolic pressure gradient across the mitral valve, the pressure difference between the left atrium and left ventricle in diastole.

C
ليش غلط
In mitral stenosis the pressure that rises is left atrial: LA pressure exceeds LV end-diastolic pressure, so the raised pressure sits behind the valve, not in the left ventricle.
