A 41-year-old man comes to the emergency department with a 2-week history of fever, anorexia, weight loss, and fatigue. He is otherwise healthy; he recently had his teeth cleaned. On physical examination: he appears ill, with a high-grade temperature and a few petechiae in both eyes. Cardiac auscultation reveals a pansystolic murmur maximal at the apex and radiating to the axilla. Blood drawn and sent for culture Which of the following is the appropriate diagnostic test to confirm the diagnosis?

الإجابة: D. Echocardiogram

✓ مطابق للمصدر CARDIOLOGY- VHD+IE.pdf · p. 17

Fever, weight loss, a recent dental procedure, conjunctival petechiae and a new MR murmur suggest infective endocarditis. Blood cultures are already drawn, so the test that completes the diagnosis is the echocardiogram, which shows vegetations (a major Duke criterion). ECG, CXR, stress test and catheterisation don't diagnose IE.

ليش كل خيار صح أو غلط

  • A ليش غلط

The ECG is listed only as a supportive test in endocarditis (a long PR interval); it does not show the vegetation, which the echocardiogram does.

  • B ليش غلط

The imaging listed for endocarditis is the echocardiogram (TTE may show vegetation above 2 mm; TOE is more sensitive), not a chest X-ray.

    ✓ Valvular disease ...pdf · p. 24
  • D ليش صح

Echocardiography is ordered for all suspected endocarditis, and echo showing vegetation, abscess or new valvular regurgitation is a major Duke criterion.
