الإجابة: C. Harsh pansystolic murmur at the lower left sternal edge
VSD causes a holosystolic (pansystolic) murmur from left-to-right shunting. It is heard at the left 4th ICS / lower left sternal border. An ejection murmur at the right upper sternal edge is AS, an apical mid-diastolic rumble is MS, an early diastolic murmur is AR, and a continuous machinery murmur is PDA.

That is aortic stenosis: ejection click plus crescendo-decrescendo murmur, heard at the right upper sternal border.

B
ليش غلط
A diastolic rumble at the apex is mitral stenosis: a decrescendo murmur from blood crossing the narrowed valve, heard at the apex after an opening snap.

C
ليش صح
VSD shunts blood from LV to RV, giving a holosystolic murmur heard at the left 4th intercostal space (lower left sternal border); a smaller defect is louder.

D
ليش غلط
An early diastolic murmur at the left sternal edge is aortic regurgitation: blood leaks back from the aorta into the LV across a dysfunctional valve.

A continuous machinery murmur is PDA: blood crosses from the aorta to the pulmonary artery in both systole and diastole, heard at the left infraclavicular area.
