الإجابة: C. Papillary muscle rupture
A few days after MI, sudden pulmonary oedema (SOB, frothy sputum) with a new pansystolic murmur at the left 5th ICS (mitral area) = papillary muscle rupture → acute mitral regurgitation. Free-wall (LV) rupture causes tamponade/collapse, not a murmur. The bank's mention of aortic regurgitation is wrong; it is acute MR.

A

Aortic dissection can cause acute aortic regurgitation, which gives a soft early diastolic murmur, not a harsh pansystolic murmur at the left 5th intercostal space.

Free wall (LV) rupture presents as cardiac tamponade (obstructive shock, JVD, muffled heart sounds), not as a pansystolic murmur with pulmonary oedema.

C
ليش صح
Papillary muscle rupture causes acute mitral regurgitation with cardiogenic shock and a holosystolic murmur heard at the left 5th intercostal space, mid-clavicular line.

Tricuspid regurgitation gives a blowing holosystolic murmur at the lower left sternal border, louder on inspiration, with right-sided failure signs (JVD, ascites), not frothy sputum.
