الإجابة: A. Murmur louder on expiration
Mitral regurgitation is a left-sided murmur, and expiration increases left-sided venous return, so left-sided murmurs get louder on expiration. MR is a holosystolic apical murmur radiating to the axilla/back; a murmur at the 2nd right intercostal space radiating to the carotids is aortic stenosis, and an early diastolic murmur suggests aortic regurgitation.

A
ليش صح
Mitral regurgitation is a left-sided murmur, and expiration raises left-sided venous return, so it gets louder on expiration (right-sided murmurs get louder on inspiration).

B
ليش غلط
A murmur in the second right intercostal space radiating to the carotids is aortic stenosis (harsh crescendo-decrescendo systolic), not mitral regurgitation.

C
ليش غلط
Mitral regurgitation is holosystolic, heard throughout systole at the apex, so it is not an early diastolic murmur.

D
ليش غلط
Mitral regurgitation is heard at the apex and radiates to the axilla, not to the carotids.
