الإجابة: A. DM
Diabetes mellitus is the classic association of atypical/silent (painless) MI, because autonomic neuropathy blunts cardiac pain signalling. The lectures list diabetics (with the elderly and post-transplant patients) as the groups with atypical presentations. Hypertension, male sex and previous MI are CAD risk factors but are not what makes an MI present silently.
